FINANCING AND INSURANCE

Here at NorthPark Family Dentistry, we want you to have an enjoyable experience while getting the dental care that you need for you and your family. We accept many major dental insurance plans, but we do not accept State/Medicaid or Health and Wellness insurance (with the exception of Hawk-I through Delta Dental).

If your dental plan doesn’t cover elective procedures, like cosmetic restorations, we happily accept alternative payment methods. Below are the payment options that we accept if you need to pay out-of-pocket.


We do all payments through www.carecredit.com Once you sign up you will get an account number. You must have your account number or your card when you come to your appointment if you plan on doing a payment plan. Otherwise your amount will be due in full on the day of your appointment. At your appointment you can choose 6 or 12 months for payment options. 



PLEASE NOTE: Consistent cancellations or missed appointments may cause you to not be seen at NorthPark Family Dentistry.


Returned Checks: NorthPark Family Dentistry charges $30 for returned checks.


Past Due Accounts: A service charge of 2.0% per month on any unpaid balance will be charged on all accounts exceeding 60 days, monthly, until balance is paid in full.




Frequently Asked Questions
    MISSED OR CANCELED APPOINTMENTS

        Canceled Appointments: If you are unable to keep your scheduled appointment, please call our office at least 24 hours in advance to cancel or reschedule. The sooner you contact us, the more time we have to provide another patient with the available time slot.


        Broken Appointments “No Show, No Call”: Your appointment time has been reserved especially for you and we strongly encourage all of our patients to keep their appointments. We reserve time in our schedule to accommodate our patients. When we have someone not show or call to cancel their appointment last minute, it is time lost that we could have provided dental care to another patient in need. A fee of $30.00 will be charged to any account that has two “no show/no call” missed appointments per account. After two or more “no show/no call” appointments or two or more last minute cancellations at your appointment time, we reserve the right to not re-appoint you or anyone on your account.


        New Patients: If you miss your first appointment with a “no call/no show” you and anyone on your account will not be re-appointed and will be restricted from being seen in our office.


    OTHER PAYMENT OPTIONS
    FINANCING

        If you need some assistance in paying for your dental care, CareCredit may be able to help. This financing option features low, easy-to-manage monthly payments, and it even has interest-free plans available to those who qualify.


        NorthPark Family Dentistry requires payment on the day of your treatment. Please remember you are fully responsible for all fees charged by this office regardless of your insurance coverage. If you have any questions, please do not hesitate to ask. We are here to help you get the best treatment that you need!

    DENTAL INSURANCE

        It is YOUR responsibility to provide us with the correct, current insurance information for you/your family. You are responsible for co-insurance, deductibles, or non-covered services not paid by your insurance company. 


        Primary/Secondary Dental Insurance: If you have two Dental Insurance policies, it is your responsibility to know which one is Primary and which one is Secondary. This will need to be verified at least 24 hours prior to your appointment, or we will need to reschedule.  


        We will give you your estimated payment at check-out prior to your services going through insurance. That will be due at the time of service. This is only an ESTIMATE so after insurance is processed there might be an additional amount due. 


        We will then file your insurance claim as a courtesy, provided we have a copy of your insurance card. There may be times when you have a remaining balance after your insurance carrier has paid their portion. In these instances, you will receive a statement from us in the mail.


        If we do not receive payment from your insurance carrier within 60 days, you will receive a statement from us in the mail. You are then responsible for payment of your treatment fees and collection of your benefits directly from your insurance carrier. Please be aware that it is ultimately your responsibility to understand your insurance benefits, including any waiting periods, limits, and exclusions.
